Safeguarding concern

Please use this form to report a Safeguarding Concern about an adult.
 
(Chapter 14 of the Care Act 2014) The safeguarding duties apply to an adult who:
 

• Has needs for care and support (whether or not the local authority is meeting any of those needs)
• Is experiencing, or at risk of, abuse or neglect
• As a result of those care and support needs is unable to protect themselves from either the risk of, or the experience of abuse or neglect

 
A 'safeguarding concern' is when any person has reasonable cause to think an adult with needs for care and support, is experiencing, or at risk of, abuse or neglect and is unable to protect themselves because of those needs (Adult at risk)

Information collected in this referral form will need to be shared with the agencies identified in the form to help us provide the services you need.
 
We will treat your information as confidential and will only share it if we are required to do so by law, or unless you (the adult at risk) or someone else may come to harm if we do not share it. We will only share the minimum information required to keep you or others safe. Information collected in this referral form may need to be shared with the agencies identified in the form, or other agencies, to help us provide the support you (the adult at risk) need.
