You are initially presented with 3 questions which relate to the Referrer

However, you will need to register or sign in (if already registered) to progress further with this page/form.
Once logged in additional questions will be displayed, ie:
Details of person completing this form – these questions also relate to the Referrer
Further Details – these questions relate to the Adult at Risk
You are completing this section of the form on behalf of the Adult at Risk, so unless you are the Adult at Risk, you should select ‘Someone else' or ‘Someone else in a professional capacity.'
The questions which follow relate to the Name, Date of Birth, Gender, Ethnicity and Address Details of the Adult at Risk. The system requires a date of birth for the Adult at Risk to be added. If you do not know the Adult's date of birth select estimate DOB and add in 01/01/1900.
Safeguarding Concerns must be reported
IMMEDIATELY in order to safeguard the Adult at Risk.
